Appointments can be made by calling our office during normal business hours at 1.833.MH4LIFE(644.5433).
To provide the highest quality of care to our patients. ALL Appointments must be confirmed at least 24 hours in advance of your scheduled appointment date and time, or your appointment will be canceled. Thank you for your understanding.
After Hours & Emergency Care
If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7-days a week.
Prescriptions are renewed during normal office hours. In some instances, we may require that the patient be seen in our office prior to medication renewal.
All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.
Preparing for Your Appointment
Please bring the following items with you:
- All medications you are taking
- Picture ID (driver’s license, military ID, etc.)
- Insurance cards
- Completed new patient and consent forms
- ID card if you already have a stent, pacemaker/ICD, artificial valve, etc.
Our office accepts most insurance plans. We are happy to discuss billing and insurance in a pre-appointment phone call.
